10 year warranty available on Silestone kitchen worktops installed between october 2008 - march 2009 through an approved fabricator.

For this form to be accepted.

* All fields must be filled in clearly and completely and information must be accurate.
* The form must be filled in and sent within 30 days of Silestone worktop installation.
